Joint Providership
Joint providership is an opportunity for MPA, an ACPE-approved provider, to work closely with another organization to design, develop, implement and accredit quality continuing pharmacy education programs.
Please be sure to read through the Introduction first, as it will provide you with an overview of what’s involved in the process. Please note that this is a large file and it may take a few minutes to download.
NOTE: There will be a $2 per participant price increase effective Jan. 1. 2024. If you would like additional information or have any questions about the joint providership process, please contact MPA.

Individual Accreditation
Use these forms if you only have one activity or will be accrediting activities within your event individually.

⇒Presenter Faculty Agreement (completed by presenter)
⇒Presenter Conflict of Interest Form(completed by co-presenter(s) if activity has more than one facilitator)
Entire Event Accreditation
Use these forms if you plan to accredit the event in its entirety as one set of CEU.

Joint Providership Program Posttests
If you participated in a Joint Providership program that was accredited for continuing education through Michigan Pharmacists Association and need the online link to access the posttest and course evaluation, please contact the administrator of your program to request this information.
